Patient transport – Reimbursements and conditions
What reimbursement will you receive?
The ZEM Polis offers full (100%) reimbursement of all the costs incurred. You will qualify for this reimbursement if you use transport services of a contracted taxi company or travel by public or private transport.
Excess
Travel expenses incurred for seated patient transport are subject to your excess.
Personal contribution
A personal contribution of €118 per calendar year applies.
What costs does the reimbursement of seated patient transport cover?
You will qualify for a reimbursement of your travel expenses when you use seated patient transport. The transport must be related to a treatment covered by your basic healthcare insurance.
In addition, you may qualify for reimbursement of the travel expenses incurred by:
a person who accompanies you during the transport if such guidance is necessary;
a person accompanying a child under age 16 during transport.
NB. The costs of your companion's return journey will not be reimbursed.

Situations in which you may qualify for reimbursement
- The transport must be related to a treatment covered by your basic healthcare insurance.
- Transport to a location for consultation, treatment, examination and kidney dialysis check-ups.
- Transport to a location for consultation, treatment, examination and chemotherapy, radiotherapy or immunotherapy check-ups as part of cancer treatment.
- Transport to a location for consultation, treatment, examination and check-ups if you depend on transport over a prolonged period of time in connection with a long-term illness or condition. In such situations, the hardship clause applies.
- If you have limited vision. By this we mean less than 30% vision in your least affected eye, as attested by a doctor's medical certificate.
- If you depend permanently on a wheelchair. Your wheelchair is reimbursed under the Social Support Act (WMO), or you are able to produce a relevant medical certificate issued by a doctor. Do you have an adapted private car that was reimbursed under the WMO? In that case you will not qualify for reimbursement of the costs of seated patient transport.
- A child under age 18 who permanently needs care and nursing owing to a disability or complex physical condition (intensive paediatric care).
- Transport to and from rehabilitation care for the elderly.
- Day care treatment as part of a care programme to address a chronic progressive degenerative condition, a non-congenital brain injury or an intellectual disability (as part of Medical Care for Specific Patient Groups (GZSP)).
- Weekend leave may qualify for reimbursement if it qualifies as resocialisation leave or if the institution is closed at the weekend. You must apply for permission in advance.
What type of transport to choose?

Private transport
The reimbursement for private transport is €0.32 per kilometre. We calculate the number of kilometres based on the shortest route as indicated by the ANWB route planner. To claim expenses, use the Claim Form.

Public transportWe will reimburse costs of second-class public transport. To claim expenses, use the Claim Form.

Taxi transport
If you need taxi transport, you must apply for permission in advance. The taxi transport is provided by ZCN-Totaalvervoer; you must book the trip by calling our Transport Line. Once you have received our permission, you can make an appointment with the taxi service provider by calling ZCN-Totaalvervoer at (010) 2 808 182 (between 7:00 - 23:00).
The taxi will pick you up at your home address and take you to your care provider. No more than 30 minutes and no less than 10 minutes before the time of the appointment with the taxi provider. The taxi will pick you up at the care provider to take you home again.

What is nog reimbursed?

You go to a non-contacted transport providerIf you choose a non-contracted transport provider, the reimbursement that you receive will usually be lower. The reimbursement equals 75% (or less) of the prevailing Dutch market rate.
